Textbook & School Supply Drive Thru
Lakeview will have a school materials distribution day where every child will be expected to pick up their materials and wave to their teacher. Here are directions we need you to follow:
Display your child’s teacher name, Grade paper, and
Wear your mask.
Stay in your car.
Be Patient, Smile, & Wave to your teacher.
When?
- Monday, August 31st

8 - 9 AM: PK & Kindergarten

9 - 10 AM 1st Grade

10 - 11 AM 2nd Grade

11 AM-12 PM 3rd Grade

1- 2 PM 4th & 5th Grade
How? (Please, remain in your cars.)

Drive thru the valet line & display the name paper, with the following

Teacher name

Grade

Student Last name, first name

Don't Forget to Complete Student Data Confirmation
Please log into the parent portal and complete your child's Data Confirmation, if you haven't already done so. Please print and complete the following forms from the parent portal (please print your child's name and grade on each form):- Student Insurance Protection Form
- Student McKinney Vento Assistance Form (if applicable)
- Medication Authorization Form (if applicable)
- Asthma Action Plan (if applicable)
